New players, what killed you last?

I started about two weeks ago and was doing pretty well. I was (and still am) hooked, so in my free time, when I wasnt playing, I was watching tutorials, and when I wasn't watching tutorials, I was reading about EVE. I learned so much... but not enough. I've died three times in the last two days. My last two deaths in particular got me thinking...

There is a TON of information out there from pros on what to avoid. But very little compiled data on how this generation of new players are getting themselves killed. After watching dozens of guides on this and that, I've realized very very few of them are actually 100% complete on any subject.

Maybe by approaching this from the other direction, and hearing from the newbros themselves what has tripped them up, we might learn something.

So, either just to commiserate, or for our stories to serve as a warning to others, or for reference to those who make it their business to train up people like us, what got you killed last?
