Doc says 290 is fine?

I've been recently, seriously struggling with my libido. I'm 29, 5'11", 200lbs. I'm never in the mood and it's seriously damaging my relationship. My girlfriend has started to get really resentful, and begged me to take testosterone. I was reluctant to talk about it, because I'm not that old and deep down I know if it is my T levels, it's most likely from my poor lifestyle. Of all the things you're supposed to do to stay healthy, I currently do nearly the opposite.
I finally did get a blood test from the VA, and the doc said he highly doubted that the issue is from my T levels and said that having sex once a week was normal.. He told me he'd call me if the test came back out of range.I never got a call back so I didn't think much about it. Well, tonight was my anniversary, and the issue with sex resulted in an argument and I remembered I could see the test result online. What I found was confusing. They measure in ng/ml while the common measurements are nmol/dl or ng/dl. Not a big issue, just confusing. When converted to ng/dl my number is **290**, which is just barely in their reference range of 280-800 (reference from 2002 btw). This number is also below the minimum of almost every other scale I've found online tonight..
My questions are: How bad is it? How big of a difference does even a small amount like, say \~20ng/dl make in terms of mood and libido? Knowing how poor my lifestyle is, most changes should benefit me (diet, exercise) - how quickly could those changes start to recover my T, mood, libido?
