Hottub rash

We empty our hottub, sanitize, flush filter, use bromine and refill after every guest in our cabin rental. Guest stayed the weekend and says they got a bacterial rash. The only bacteria in that sparkling clean hottub in any quality was theirs. Like they didn't shower first or relieved themselves in the water. Is there something I'm not understanding or could do to prevent this in the future?
